You can totally use a premade deck (boaras is significantly better than mile down. Pick one and use just that) but just be prepared for anki sessions after UWorld.

im pretty sure there is a kaplan specific milesdown deck that you use to sync up with Kaplan. In terms of incorporating it into my readings, I would usually do the specific anki deck for a chapter after I've read and take notes on that chapter.
